Editor's Note · Issue 14

The bloodwork your doctor ordered didn't ask the questions you came in to answer.

A standard annual panel checks four to six markers. None of them index the growth-hormone axis, the melanocortin pathway, or the systemic inflammatory tone that determines whether a peptide protocol has a rational basis. We index those things.

The peptide market has run far ahead of the evidence. Compounding pharmacies, telehealth clinics, and grey-market vendors prescribe with a presumption of universal candidacy.11 Total Virility was founded on the inverse premise: that a peptide recommendation should begin with a measurement, end with a citation, and contain no commercial conflict in between.

This volume covers eleven peptides — from BPC-157 and TB-500 to the GLP-1 incretins reshaping metabolic medicine.811 Each is mapped to a defined biomarker panel. Each rests on a published trial or mechanistic study. Each comes with the contraindications the marketing copy tends to omit.

Intake & assay selection

A six-minute structured intake records your symptoms, goals, medications, and contraindications. We select the assay panel that can answer the relevant questions — not a generic preset.

Venous draw at any CLIA lab

A requisition routes to Quest or Labcorp. Walk in to any of 4,500+ patient service centers. Fasting is required for the metabolic subpanel; otherwise no preparation.

Annotated peptide report

Each flagged biomarker is annotated to the peptide literature with author, journal, and PMID. Recommendations include a candidate stack, monitoring schedule, and explicit contraindications.

5–7 days · PDF + dashboard · share with your physician
From the report

Every recommendation begins with a flagged biomarker.

The report is structured like a journal article: methods first (assays, reference ranges, sample handling), then a results section with flagged biomarkers, then a discussion mapping each flag to the peptide literature.

No flag without a citation. No recommendation without a contraindication section. No protocol without a monitoring schedule.

§ Discussion

Patient's IGF-1 sits at the 11th percentile for age-matched controls. A GHRH analog stack (CJC-1295 + ipamorelin) is supported by Teichman 200613 with pulsatile preservation per Ionescu 200614. See dosing windows, p. 7.

I'd been on telehealth-prescribed semaglutide for nine months. The report flagged my lipase trending up and recommended a wash-out window. I'd have missed it.

My IGF-1 came back at 96. I'd been considering an "anti-aging stack" from a clinic — turned out my pituitary had a reason worth investigating first.
J.R., 41·Growth axis · Issue 09
The contraindication section alone is worth the price. BPC-157 plus my baseline hsCRP, with the trial data inline — it changed the conversation with my sports-med doc.
D.S., 29·Recovery panel · Issue 13
I appreciate that the report tells me what isn't supported by evidence as clearly as what is. Melanotan II got a hard pass and a reason why.
